Senator Takubo moved to amend the amendment on page 5, section 4, lines 12 and 13, by striking out “subsections (f) or (g) or attends virtual public school pursuant to subsection (i) of this section” and inserting in lieu thereof “subsections (g) or (h) or attends virtual public school pursuant to subsection (j) of this section”;

On page 6, section 4, after line 53, by inserting a new subsection (f) to read as follows:

(f) A physician, physician assistant, or nurse practitioner who provides any person with a false certificate of immunization against chickenpox, hepatitis-b, measles, meningitis, mumps, diphtheria, polio, rubella, tetanus and whooping cough is guilty of a misdemeanor and, upon conviction, shall be fined not less than $25 nor more than $100.;

On page 6, section 4, line 54, by striking out “(f)” and inserting in lieu thereof “(g)”;

On page 6, section 4, line 59, by striking out “(g) Subsections (b) through (e)” and inserting in lieu thereof “(h) Subsections (b) through (f)”;

On page 7, section 4, line 70, by striking out “(h) A form signed pursuant to §16-3-4(g)” and inserting in lieu thereof “(i) A form signed pursuant to §16-3-4(h)”;

On page 7, section 4, line 74, by striking out “(i) A full time virtual public school student who does not physically attend school shall be exempt from the requirements of subsections (b) through (e) of this section.” and inserting in lieu thereof “(j) A full time virtual public school student who does not physically attend school shall be exempt from the requirements of subsections (b) through (f) of this section.”;

On page 7, section 4, line 76, by striking out “(j)” and inserting in lieu thereof “(k)”;

On page 7, section 4, line 79, by striking out “(k)” and inserting in lieu thereof “(l)”;

On page 7, section 4, line 82, by striking out “subsections (f) or (g);” and inserting in lieu thereof “subsections (g) or (h);”;

On page 7, section 4, line 84, by striking out “subsections (f) or (g).” and inserting in lieu thereof “subsections (g) or (h).”;

On page 7, section 4, line 85, by striking out “(l) The report required under subsection (k) shall:” and inserting in lieu thereof “(m) The report required under subsection (l) shall:”;

And,

On page 8, section 4, line 89, by striking out “(m) No personal identifying or personal health information of any individual shall be included in the report required under subsections (k) and (l) of this section.” and inserting in lieu thereof “(n) No personal identifying or personal health information of any individual shall be included in the report required under subsections (l) and (m) of this section.”.
